Welcome to Lakewood High School!For your convenience, we have provided the list of documents required to enroll your child in Lakewood High School. An enrollment packet is also available at the high school. When you have all documents completed and are ready to schedule an appointment with the principal, please call the high school office at 616.374.8868. 2. Transcript of grades: Please obtain an unofficial copy of a transcript from your former school as well as current grades, if applicable. 3. Immunization Records: State law requires that we have up-to-date immunizations for all new students, or a signed waiver stating the reason why immunizations are not current. The enrollment checklist includes a list of required immunizations. If you require an immunzation waiver, downloads are available below. 4. Birth Certificate: State law requires that a certified birth certificate (with the raised seal) be presented upon school enrollment. Certified birth certificates are available from the county clerk in the county in which your child was born. If your cihld was born out of the state of Michigan and you need assistance in obtaining a certified copy, please contact Central Office at 616.374.8043. 5. Special Education: If student received special education services in former school, please obtain a copy of the current IEP and assessment report. All of the following enrollment forms must be completed prior to meeting with the principal.
